I UNDERSTAND AND ACKNOWLEDGE I AM NOT REQUIRED TO GIVE THIS AUTHORIZATION AND CONSENT AS A CONDITION OF PURCHASING ANY PROPERTY, GOODS OR SERVICES OR RECEIVING ANY LOAN OR FINANCIAL SERVICE. I acknowledge I may withdraw and revoke this authorization and consent by written notice to the Credit union at Community First Credit Union of Florida at P.O. Box 2600, Jacksonville, FL 32232, by email to CommunityFirst@c1cufl.org, by telephone at 800.342.8416 or by any other reasonable means.

I represent and agree I am the subscriber or customary user with respect to the telephone number provided and have the authority to give this consent. By selecting yes and providing a phone number to this online consent, I further agree to notifiy the Credit Union of any change to the telephone number provided. I agree to identify the Credit Union, its directors, officers, employees and agents, and hold them harmless, from and against any and all losses, claims, damages, liabilities, costs or expenses (including any attorneys' fees) that arise out of my breach of any of the foregoing representations and agreements.
